Patient Appeals
Request a Case Screening
First check whether the matter fits EUC’s current service. The screening questionnaire opens in Microsoft Forms; no email request is needed.
Before you start
- You are an Illinois patient with a commercial or employer-sponsored health plan.
- The denial involves a medically substantive issue within available expertise.
- You have checked the deadline in the denial notice.
Medicare, Medicaid, workers’ compensation, disability benefits and property/casualty claims are outside the current service. Routine billing corrections and primarily legal disputes do not fit this service.
Open the screening questionnaire
Microsoft hosts the EUC form. It asks for patient and contact identifiers, plan information, limited details about the denied service, dates, deadlines and prior review. Those answers may reveal health-related information and are used for scope, conflict and deadline screening and communication about your request.
Provide only the requested screening details. Do not upload full records, denial letters, a full medical history, payment details, Social Security numbers or insurance member IDs. Document-return instructions are provided separately after preliminary clearance and required authorization.
Website inquiries and confidential information
Open Screening QuestionnaireYou will leave this website for Microsoft Forms. Submitting the form does not mean EUC has accepted the matter or agreed to file an appeal.
After screening
If a clinician, caregiver, attorney or another service is already handling the appeal, tell EUC during screening so responsibilities can be clarified before any engagement.
EUC evaluates expertise, availability, conflicts and time remaining. Matters that may proceed receive engagement and authorization materials with separate upload instructions. Substantive work starts only after EUC confirms the written scope and required engagement conditions.
Protect your deadline: screening, payment or signed documents do not extend it. Until EUC confirms responsibility for a specific submission in writing, continue to follow the plan’s instructions yourself. For urgent care, contact your treating clinician and plan; for emergencies, use emergency clinical services.
