Choose a service and stage for general preparation guidance. This does not retrieve your case status or record your selections.
Already working with EUC?
Use the matter-specific instructions EUC sent for returning agreements, records and other documents. An internal SharePoint library or a link for another client is not an authorized upload destination.
Open the approved Microsoft return link or payment link in your own engagement correspondence. If it is unavailable, ask for the appropriate instructions through a nonconfidential inquiry. Do not put records, denial letters, case identities or account numbers in the general form.
There is no general-purpose upload or live case-status login on this page. EUC confirms acceptance, scope, responsibility and next steps directly.
Prepare for your next conversation
This optional checklist is for your own preparation. It resets when you leave the page and is not sent to EUC.
Other pathways
Clinicians & referring practices
Direct a patient or appropriate representative to the Patient Appeals screening pathway. A referral alone does not authorize records access or create an engagement.
Patient Appeals screening →Former EUC clinic patients
The EUC clinic closed February 17, 2026. Records and continuity-of-care instructions remain available.
Former-patient instructions →Keep responsibilities clear
Screening does not extend a deadline or mean that EUC will make a submission. Follow your plan or matter instructions until EUC confirms its responsibility in writing. Patient Appeals provides medical review and administrative appeal support, not clinical treatment or legal representation.
