SimplePractice is the exclusive platform for the storage of all medical records, including treatment notes, prescriptions, and visit documentation.
All appointment scheduling, confirmations, and secure messaging are handled through SimplePractice. Telehealth video visits are conducted exclusively through SimplePractice's HIPAA-compliant platform.
All payments are securely processed through Stripe, Maverick, or PayPal (PCI-compliant).

CONSENT FOR TELEHEALTH CONSULTATION

I understand that my health care provider wishes me to engage in a telehealth consultation.
My health care provider explained to me how the video conferencing technology that will be used to affect such a consultation will not be the same as a direct client/health care provider visit due to the fact that I will not be in the same room as my provider.
I understand that a telehealth consultation has potential benefits including easier access to care and the convenience of meeting from a location of my choosing.
I understand there are potential risks to this technology, including interruptions, unauthorized access, and technical difficulties. I understand that my health care provider or I can discontinue the telehealth consult/visit if it is felt that the videoconferencing connections are not adequate for the situation.
I understand that a telehealth consultation by nature cannot capture a patient's full clinical picture in every scenario. For example, there are physical findings that cannot be assessed (e.g., prostate exam). As such, some clinical conditions cannot be fully assessed.
I have had a direct conversation with my provider, during which I had the opportunity to ask questions in regard to this procedure. My questions have been answered and the risks, benefits and any practical alternatives have been discussed with me in a language in which I understand.

EXPERT UROLOGY CONSULTING PLLC
Your clinician uses a digital Note Taker to create an accurate and timely record of your care. Instead of writing notes by hand, the session will be recorded which allows clinicians to give you their undivided attention during your time together. This means better care and more meaningful conversations between you and your clinician.
Some states have two-party consent for audio recordings, so it's important for you to know that your voice and conversation with your clinician are recorded to document the appointment.
As soon as the audio is transcribed (usually a few seconds after the appointment ends), the audio recording is permanently deleted.
The recording process complies with the Health Insurance Portability and Accountability Act (HIPAA).
If you still have any questions or concerns, your clinician would be happy to discuss this with you. You have the right to withdraw your consent at any time (even temporarily).

At Expert Urology Consulting, I am committed to providing you with the best possible care. To help me manage my practice efficiently and enhance my services, I use technology, including certain artificial intelligence (AI) tools. This document explains how I use these tools and asks for your consent to use them as part of your treatment. Your privacy, confidentiality, and the quality of your treatment remain my highest priorities.
AI tools are used strictly for administrative and supplementary support tasks under my direct supervision. These tools do not provide medical care, make independent clinical decisions, or interact with you directly. The specific purposes for which I may use AI now and in the future include:

Right to Revoke Consent:
Your consent is voluntary. You have the right to withdraw this consent at any time by notifying me in writing. Revoking your consent will not affect your ability to receive medical services.

Please remember to cancel or reschedule 24 hours in advance. You will be responsible for the entire fee if you need to cancel or reschedule within 24 hours. If Dr. Wille needs to reschedule your appointment at any time, you will not be charged. If your appointment needs to be canceled because of our office's doing, you will receive a full refund. If you are late for a session, you may lose some of that session time. The standard meeting time for new patient appointments is 30 minutes. The standard meeting time for follow-up patient appointments is 15 minutes.
A $35 service charge will be charged for any checks returned for any reason for special handling.
If you need to contact me by phone, please call the office. You may leave a message on that voicemail. EUC does not employ any other staff to answer the phone. I am often not immediately available; however, I will attempt to return your call within 24 hours. If a true emergency situation arises, please call 911 or any local emergency room.
As a patient of Expert Urology Consulting, you will have access to the patient portal. Electronic communication is preferred and has a much faster return speed. It is through this portal that we can message securely. I cannot ensure the confidentiality of any form of communication through other electronic media like including text messages. If you prefer to communicate via email for issues regarding scheduling or cancellations, I will do so. Please allow 1 business day for a response to electronic communication, although response time is often much faster. While I may try to return messages in a timely manner, I cannot guarantee immediate response. This method should not be used to communicate in an emergency. Services by electronic means, including but not limited to telephone communication, the Internet, facsimile machines, and e-mail is considered telemedicine. If you and your physician chose to use information technology for your treatment, you need to understand that:
Treatment decision making and laboratory review cannot be done over the portal. Please schedule an appointment with Dr. Wille to review any laboratory results or to discuss treatment decision-making.
If you are a minor, your parents may be legally entitled to some information about your treatment. I will discuss with you and your parents what information is appropriate for them to receive and which issues are more appropriately kept confidential.
Ending relationships can be difficult. Therefore, it is important to have a termination process in order to achieve some closure. The appropriate length of the termination depends on the length and intensity of the treatment. I may terminate treatment after appropriate discussion with you and a termination process if I determine that the treatment is not being effectively used or if you are in default on payment. I will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose of terminating. If treatment is terminated for any reason or you request another provider, I will provide you with a list of qualified Urologists to treat you. You may also choose someone on your own or from another referral source. Should you fail to schedule an appointment for three consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, I must consider the professional relationship discontinued.
This practice does not service any in-person clinic, hospital, or emergency room. Should you need this type of care, EXPERT UROLOGY CONSULTING will not be able to manage your clinical needs and you will need to be under the care of the providers who are credentialed and staff those entities.

Important Insurance Information
It is your responsibility to ensure your plan covers diagnoses and services that we will be addressing. We bill your insurance only as a courtesy. We would urge you to check with your insurance company by calling the number on the back of your card to make sure the diagnosis you are seeking treatment for is covered by your plan (many plans do not cover erectile dysfunction, for example)! If your insurance company asks for the CPT codes for the appointment, we use 99205 (new patient) and 99215 (established patient) visit codes. The Billing NPI for Expert Urology Consulting PLLC is 1578364683. Please indicate that we are a TeleMedicine (NOT TeleHealth) provider. Some plans do not cover TeleMedicine.
